TripleCare is a telemedicine-enabled physician service, combined with video communication technology that allows two-way, real time interactive communication between the patients and the physician. During these real time interactions, a physician is able to use digital diagnostic equipment (including stethoscope, ECG, otoscope, and high resolution external skin/wound camera). eSNF physicians communicate with and examine patients live, from off-site locations for immediate triage, treatment, and follow-up.

Hospitalizations are very stressful and can lead to additional illness and complications for patients. Most hospital readmissions occur during evenings, weekends, and holidays, when a physician’s availability is limited. Our partnership with TripleCare has allowed us to step up to the challenge of reducing avoidable hospital readmissions and delivering a better care experience for our patients.
